Abstract [sv]

Bakgrund: Patienter med svår psykisk ohälsa har en förkortad livslängd samt ökad risk för fysisk ohälsa relaterat till ohälsosamma levnadsvanor. Bristen på specialistsjuksköterskor inom öppenvården ökar kravet på allmänsjuksköterskan kompetens. Trots befintliga interventioner och kunskap hos sjuksköterskor gällande patientgruppens ökade risk för fysisk ohälsa återfinns brister inom organisation och dagligt arbete. Syfte: Syftet var att belysa sjuksköterskans upplevelser av hälsofrämjande omvårdnad hos patienter med svår psykisk ohälsa inom öppenvården. Metod: En allmän litteraturstudie baserad på nio vetenskapliga resultatartiklar av både kvalitativ- och kvantitativ metod. Resultat: I litteraturstudien presenteras fem huvudkategorier: Relationsskapande, kunskaper och erfarenheter, resurser och prioritering, samarbete och kommunikation samt följsamhet och utmaningar. Relationen mellan sjuksköterska och patient var en grundläggande förutsättning för hälsofrämjande omvårdnad. Sjuksköterskor upplevde utmaningar inom hälsofrämjande omvårdnad av fysisk ohälsa i relation till bristande resurser, kunskap samt svåra bedömningsverktyg. Konklusion och implikation: Bristande klinisk kompetens hos allmänsjuksköterskor lyfter relevansen för vidare forskning som syftar till att öka kunskap och trygghet i yrkesutövandet.

Abstract [en]

Background: Patients with severe mental illness have a shortened lifespan and increased risk of physical ill health related to unhealthy lifestyle habits. The lack of specialist nurses in outpatient care increases the demand for registered nursing competence. Despite existing interventions and knowledge among nurses regarding the patient´s increased risk of physical ill-health, shortcomings are found in the organization and in daily work. Purpose: The purpose was to highlight the nurses’ experiences of health promotion nursing in patients with severe mental illness in outpatient care. Method: A general literature study based on nine scientific outcome articles using both qualitative- and quantitative methods. Results: The literature study presents five main categories: Relationship building, knowledge and experiences, resources and prioritization, Collaboration and communication as well as compliance and challenges. The relationship between nurse and patient was a fundamental prerequisite for health-promoting care. Nurse´s experienced challenges in health-promoting care of physical health in relation to lack of resources, knowledge, and difficult assessment tools. Conclusion and implication: Lack of clinical competence among registered nurses highlights the relevance of further research aimed at enhancing knowledge and confidence in professional practice.
