    New Product Development in a Medical Device Context: Managing Projects of different Novelty2016Independent thesis Advanced level (degree of Master (One Year)), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

    Healthcare is a topic that matters since it aims to ensure better well-being for people. An important and essential part of health care is medical devices since it has the potential to increase the quality of life for people with a health problem. Among the suppliers of innovation, the medical device industry is a dynamic field providing thousands of products to the market every year with the aim to enhance people's lives. However, there are many actors that influences the medical device development such as regulations that ensures that medical devices follow a specific procedure during development, at the same time buyers and end-users need to be integrated throughout the medical device design, this results in challenges during medical device development.

    This thesis focuses on new product development (NPD) and investigates how projects are managed in a medical device context. Furthermore, the thesis elaborates projects of different novelty and the influence from the characteristic of complexity. This is done with a single-case study of a case company that develop and market medical devices. The empirical findings shows that the main challenges are in the area of clinical studies and product development, furthermore, managing NPD projects in a medical device context deals with specialized knowledge that is dispersed among a group of actors which can influence the development of the medical device no matter the novelty. It was found that the difference between the studied projects was minor in terms of complexity. Though, it was noticed that the project of radical novelty had more interaction with the end-user, which can relate to uncertainty in the function of the product, as a consequence from being completely new product. As a result from the findings, the implication is that the projects can not be treated and managed similarly as a result from uncertainty, thus, it depends on the integration of actors, consequently, influencing time of development and resources.

    This thesis contributes to the community of companies operating in a medical device context where there is minor focus on complexity in projects, it was found that it might be beneficial to make distinctions in complexity characteristics when identifying challenges and addressing NPD projects in a medical device context.

    Barn som ska opereras upplever den okända situationen som stressfylld och obehaglig. Denna litteraturstudies syfte var att belysa vad anestesisjuksköterskan kan göra för att minska oro och rädsla för barn och föräldrar i samband med anestesi. Efter analys av 16 artiklar blev resultatet fyra kategorier; information, åtgärder i förberedelsefasen, åtgärder vid anestesitillfället och bedömning av oro. Informationen ska vara omfattande och detaljrik och även beröra den postoperativa smärtlindringen. Internetprogram kan vara bra komplement till traditionell information. Goda resultat ses med åtgärder i förberedelsefasen. De kan vara utformade på olika sätt, vilka alla syftar till att öka delaktighet och reflektionsmöjligheter hos föräldrar och barn. Olika metoder kan användas i den direkta omvårdnaden som att förbättra vårdpersonalens bemötande av barnen. Ett annat är att använda avledande moment i form av clowner. För bedömning av oro kan barnens och föräldrarnas beteende bedömas i realtid. På så sätt identifieras oroliga barn och föräldrar. Resultatet i studierna visar på att oroliga barn vill ha mer omfattande information inför anestesi och operation än vad som ges idag. Att åldersanpassa och individanpassa information och att arbeta med perioperativ dialog är sätt att kvalitetshöja omvårdnaden.

    Abstract [en]

    Background: Family-centered care in family rooms is more common in Swedish neonatal care units nowadays. Infants were previously cared for in open bay, where multiple newborn children were cared for. In order for the pediatric nurse to be able to provide patient-safe and good family-centered care and obtain a good working environment, it is important to examine the pediatric nurse´s experience of working in family rooms. Aim: The aim of the study was to illustrate the experiences of pediatric nurses working with family-centered care in family rooms at a neonatal care unit. Method: The study was conducted as a literature review with systematic search and examination and builds upon nine scientific articles. The contents were reviewed according to the Bettany-Saltikov and McSherry method. Results: Pediatric nurses experienced that working in a family room promoted family-centered care. Pediatric nurses experienced a worsened work environment. A large part of the work was done alone, and it could give them a sense of isolation. They perceived that their time was no longer enough, and it led to stress and anxiety. Conclusion: Work in family rooms is a new way of working for pediatric nurses. It takes time getting used to the new operating procedures for family-centered care in family rooms to be good and patient safety, simultaneously to maintain a good work environment.

    Abstract [en]

    The father is central to the child´s development, but knowledge and understanding of the father´s experiences of his role are lacking. The purpose of this literature study was to explore and describe the man´s experiences of his role as a father to children under six years old. The study resulted in 13 qualitative articles where five themes emerged: dealing with biological differences, passing on masculinity, responsibility as a family provider and decision-maker, striving for equality and involvement, and emotional and practical connection. The result consists of articles from several different countries: Norway, USA, Canada, Ireland, Finland, Czech Republic, Thailand, Poland, Sweden and Ghana. The geographical place and context tend to be significance for the experience´s men have of the father role. Society requires awareness and openness to how different fathers experience their role, knowledge may also be of importance to students in various health care educations who may come to meet fathers in the child health care. Suggestions for continued research may be how same sex couples experience their parenting roles.

    Abstract [en]

    Vaccination of typical childhood diseases in the world has as an intervention itself saved millions of people the last 200 years. Despite this the anti-vaccine movement is a growing phenomenon. The aim of this review was to map out the different factors affecting parents in their decision-making regarding childhood vaccination. 18 peer reviewed articles were analyzed and constituted the basis for the result. From this three themes emerged, all of them with inspiration from Rogers (1995) Diffusion of Innovation theory. The themes are: The Innovation, Communication Channels and A Social System. Some of the reasons for why parent choose to vaccinate their children were following a tradition/norm and/or a fear of the vaccine preventable diseases. The most prominent factors to why parents chose to refrain from childhood vaccinations were a fear of side effects and permanent disability as well as the belief that an infant is too young and its immune system is too vulnerable. An information aspect permeated all studies and the primary problem seemed to be inadequate and unreliable information from authorities and health care providers. Therefore, as a nurse, there is a need to be open to take part of the factors behind parents’ childhood vaccination beliefs to be able to help them make an adequate decision.

    Abstract [en]

    Multiple myeloma is a cancer illness that arises in the bone marrow. In Sweden 500-600 people per year get diagnosed with multiple myeloma. The purpose of the literature study was to highlight the experiences of individuals with multiple myeloma through the six dimension of pain. The study is based on a theoretical frame of reference. To get at overall picture of the individuals experiences of living with multiple myeloma, the result was described through the six dimensions of pain. The result shows that multiple myeloma patient´s social life becomes reduced, their habit changes and booth their physical and psychological health reduces. Pain, fatigue and infections caused by lower immune system affects the individuals. The knowledge of the illness in society is trivial and this makes the understanding for people with multiple myeloma and their illness slight. A proper pain assessment is necessary for the nurse to complete, to get a full understanding about the individuals and their quality of life. More research about the illness, treatments and medications is needed and the goal should be to cure the disease.

    Infertilitet är ett globalt problem som drabbar ungefär 186 miljoner kvinnor världen över. Infertilitet medför ett stort lidande och påverkar kvinnor psykiskt, fysiskt, kulturellt och emotionellt. Då sjuksköterskan har ett ansvar att upptäcka och lindra lidande är det av relevans att ha kunskap om kvinnors upplevese av infertilitet. Syftet i litteraturstudien var att belysa kvinnors upplevelse av infertilitet. Metoden i studien är en litteraturstudie där innehållet utgörs av elva vetenskapliga artiklar. I resultatet speglas kvinnors upplevelser av infertilitet och delas in i emotionella upplevelser vid infertilitet, upplevelse av okvinnlighet, upplevelse av social isolering och upplevelse av kulturell och religiös påverkan vid infertilitet. Att få barn ses som en naturlig och fundamental del i livet. Infertilitet innebär därför ett sorgearbete och en krisreaktion hos kvinnor. Infertila kvinnor upplever ofta känslor av att vara misslyckade och odugliga, då de inte kan få biologiska barn. Då infertilitet har en stark påverkan på kvinnors hälsa bör kunskap om infertilitet tillämpas både i utbildningen och i vården. Då samhället är i ständig förändring kan detta inverka på kvinnors syn på fertilitet och därmed upplevelse av infertilitet. Kunskapen kan då förebygga och lindra kvinnans lidande av infertilitet. Det krävs därför fortsatt forskning kring kvinnors upplevelse av infertilitet. Det behövs särskilt ny forskning på svenska och europeiska kvinnor, då det är en brist på forskningsområdet idag.

    Abstract [en]

    As digitalization increases in speed and we become more digital, changes are brought upon every part of society, not least our workplaces. While these changes often create efficiency in organizations, they also come with high demands on individual learning and well-being. Leaders play a central role in change processes, thus improvingleadership is of utmost importance. The purpose of this study was to examine leaders’perception of job demands, resources and learning in an organizational digital change process and contribute to the body of research with understanding of what creates success and health promotion in such change processes. To examine and answer the research questions, eight semi-structured interviews were held with leaders from the care office of the municipality. The interviews were transcribed and analyzed using conventional qualitative content analysis. The results from this study show that theleaders’ positive attitude towards digitalization induce wellbeing and that individualmotivation drivers are important resources to support healthiness. The study also finds that the most prominent demand that leaders experience in the digitalization process is to be a good role model. Further, the results show that occasions for education and reflection regarding digitalization are main drivers to promote leaders’ learning.Success is created from having a positive attitude, individual motivation drivers, a supportive learning environment for formal learning, and opportunities for informal learning. For an organizational digital change process to be health promotional, motivation and support are important factors as they create opportunities for improvement of well-being and learning at the workplace.

    Depression är idag ett av de största folkhälsoproblemen. Antalet insjuknade i depression ökar ständigt. Ungefär 25 % av alla kvinnor och 15 % av alla män beräknas någon gång få en depression som kräver behandling. Som sjuksköterskor kommer vi i vården att möta patienter med depression och för att kunna ge en så adekvat vård som möjligt behövs kunskap om hur patienten upplever sin situation och vården som erhålls. Ges inte rätt och passande behandling finns risk för försämring. Syftet med studien var att belysa patienters upplevelse av vård vid depression. Studien utfördes som en litteraturstudie där elva kvalitativa artiklar valdes ut till att ingå i resultatet. Resultatet påvisade att patienterna upplevde stöd som en betydelsefull faktor i vården. I de fall där patienterna mötte personal som var oförstående upplevde patienterna sig kränkta och hade svårare att acceptera sjukdomen och ta till sig behandlingen. Patienterna ansåg att en anpassad och tillräcklig information var av stor vikt för att kunna hantera sin depression. Information innebar även att patienten kunde vara delaktig i beslutsprocessen och ta kontroll över situationen. Vidare forskning inom området hade varit önskvärd för att kunna utveckla vårdarbetet och för att kunna möta patienterna på bästa sätt.

    Abstract [en]

    To be a parent of a child who is being cared for in hospital is complicated and difficult and affects each parent differently. Isolation can cause stress to the parents and lead to a further need for control, information and communication. The purpose of this study was to describe how parents experienced staying in a hospital isolation room with their children when the children were being cared for in hospital. The study has a descriptive qualitative design. The study showed that the parents felt awkward and worried about asking for help when they needed it. The parents placed an importance on being seen by the staff and there was a lack of information and communication. There was no information available about how to receive help, the planning process and also how to keep oneself occupied. The isolation rooms were described as sterile, cold and lacking a homely feel. Having something to do made the isolation easier to cope with however the parents still found it difficult to keep the children entertained. The parents felt trapped when they were not able to leave the room which resulted in a constant focus on the health of the child and this in turn led to an increased anxiety level. The level of help given to the parents varied and support was considered important. Further research into the importance of help available for parents, as well as how staff members could change the awkwardness parents feel, would be useful for parents, children and staff members, and may encourage new routines.

    Abstract [en]

    Every year roughly 15 million children are born prematurely. A premature born child is in need of specialized care focusing on his or her well-being. This may cause parents' involvement in the care of their child to be overridden and the possibility of connection becomes impaired. The aim of this literature study was to describe parents' experiences of coming home with their premature born child. A general literature study was conducted with systematic searches in databases and supported by content analysis. The result was presented in three main themes: Experiences of coming home, Experiences of caring for one's child at home and Experiences of support. The result showed that feelings like positivity and relief were predominant, but parents also experienced insecurity and concern. At home everyday life was characterized by new routines and various obstacles. Parents' received support from the nurse, support groups and relatives. The support from the nurse was considered important for parents to experience homecoming as positive. This literature study can give the nurse an increased understanding of parents' experiences of coming home with their premature born child and lead to better support from the nurse in those areas where the insecurity among parents' is the greatest.

    Alla har någon gång upplevt ensamhet men ensamheten är så mångfasetterad att den kan upplevas på flera olika sätt, både som ett lidande men också som en vilsam och behaglig känsla. Ensamheten ses som ett angeläget folkhälsoproblem men kan vara svår att upptäcka eller känna igen då det är svårt att sätta ord på. Syftet med studien var att belysa patienters upplevelse av ensamhet. Metoden som användes var en litteraturstudie baserat på tolv artiklar vars resultat analyserades och sammanställdes. I resultatet identifierades tre teman: Sociala relationer och ensamhet, Sjukdom och ensamhet och Känslor i samband med ensamhet. Ensamhet kan upplevas i sociala relationer men också i vårdrelationer beroende på hur sjuksköterskantar sig tid att bemöta och informera. För att bättre möta patientens omvårdnadsbehov kan sjuksköterskan använda sig av NANDAs omvårdnadsdiagnoser. Det är angeläget att arbeta fram en tydligare definition om vad ensamhet står för så att sjuksköterskan lättare ska kunna identifiera ensamhet hos patienten.

    Abstract [en]

    Previous studies on power have shown the relevance for athletic performance. Furthermore, there are several studies in plyometric training that demonstrate improvement in elasticity in sprint performance. However, few studies have been made on plyometric training for swimmers. We find it startling that no further studies have been made according to the positive effects that have been shown in swim performance. 

     

    The aim of this study was to investigate if the swimmer can increase in power output by performing plyometric training for eight weeks in link with regular exercise. In this study we used a test group of seven male and three female elite junior swimmers from Laxen, Halmstad.

     

    The tests conducted were 1RM squat, long jump, squat jump and the time between start and slide phase. We analyzed data with Dartfish. The training program was carried out twice a week for eight weeks to increase power output. The exercises were one leg bounding, truck jump, countermovement jump with medicine ball and vertical jump with weight.

     

    The improvements were significant in three of four tests. Mean pre-test of 1RM was 86.7 kg (± 16.8) and post-study had average increased to 95 kg (± 10.8). The results showed a significant difference (p = 0.05). Mean pre-test on the squat jump was 36.4 inches (± 5.5) and post-test had average increased to 41.8 cm (± 6.3). The results showed a significant difference (p = 0.001). Mean pre-test on the long jump was 2.42 meters (± 22.5) and post-test had average increased to 2.64 m (± 23.3). The results showed a significant difference (p = 0.001). The swim start showed no significant difference but the time was reduced by 9.2 percent. The greatest difference was in squat jump, which had an increase of 20 percent.

     

    The conclusion was that the swimmers gained a greater force after the training period. The tests in 1RM, squat jump and long jump had a significant difference. The swimmers had an increase in performance at the start of 9,2 percent. Although it was not a significant difference in the start, each percent increase in elite swimming is important.

    Hjärtinfarkt är den dominerande dödliga sjukdomen i Sverige, vilket fler kvinnor än män avlider i. Kvinnor insjuknar senare i livet och upplever fler och mer diffusa symptom än män. Stress är en vanlig förekommande riskfaktor, vilket kan skapas från samhällets förväntningar på kvinnor. Det finns förväntningar om omsorgsfulla kvinnor som samtidigt ska prestera bra både på arbetet och i hemmet, vilket genererar stress. Förväntningar från samhället och kroppens biologi skiljer sig mellan män och kvinnor. Män har historiskt sett representerat normen för människan i forskning och därför är det viktigt att uppmärksamma kvinnors erfarenheter. Syftet med studien var att belysa kvinnors erfarenheter av livet efter hjärtinfarkt. En litteraturstudie användes som metod, vilket genererade tio vetenskapliga artiklar som resulterade i tre teman och nio kategorier. Kvinnornas erfarenheter visade att hjärtinfarkt medförde ett avbrott i livet och med hjälp av närstående samt egna reflektioner lärde kvinnorna känna sig själva igen. En ny prioriteringsordning skapades med fokus på kvinnornas hälsa och liv. Studien visar att stress är en stor faktor i kvinnors erfarenheter efter hjärtinfarkt och att kvinnors erfarenheter behöver uppmärksammas för att vårdpersonal ska kunna ge adekvat vård. Det hade varit betydelsefullt om framtida forskning hade uppmärksammat hur stress skiljer sig mellan könen vid hjärtinfarkt, eftersom stress är en riskfaktor.

    In Sweden there are currently 7000 children and young people with the chronic disease diabetes. Young people with type 1 diabetes have sometimes tended to experiment and take risks by testing limits to develop their knowledge. Risk-taking in a chronic disease may involve medical consequences for young people. A good health care is therefore of considerable importance. A systematic literature search with a deductive approach carried out where the purpose was to describe the school nurse support to young people with diabetes type1, based on Orem´s theory of supportive and educational care system. The research articles were searched for in database Cinahl and Pubmed and 16 articles were found who answered the purpose. Dataprocessing was carried out in the form of thematic analysis.

    The result showed 4 categories of support that young people with diabetes type 1 is in need of. Based on Orems theory of supportive and educational caresystem divided into: support -physically and mentally, guidance, education and create a developed environment. The physical support consisted of having a present school nurse who was situated at school and gave support to young people with diabetes related problems when they needed help. The mental support consisted of having a good listener who understood young people's life situation and feelings. The guidance support consisted in having a written care plan for young people to be able to manage diabetes in school. Educational support for young people with diabetes their teachers and other school staff was considered important. Young people with diabetes should be able to manage their disease and care required a developed environment. A supportive environment was necessary also time and availability of food. It was important that more staff at the school would have knowledge and preparedness in the event of any emergency situations that could arise. The knowledge that was found is important in a school nurse clinical work, to be able to give adolescents with diabetes type 1 safe environment during school time. Further qualitative research is needed to improve the school nurse´s supportive service for young people with chronic illness. Methods to create a customized and secure environment for young people with chronic illnesses need to be developed.

    Background: In ballistic sports like golf power production in rotational movement play a major role for performance. To hit the ball far, high clubhead speed is crucial in golf and rotational power (medicine ball throws) have shown to have a good correlation to clubhead speed. A lower golf handicap has also shown to correlate well with higher clubhead speed. Few sport specific power tests have been executed and training and testing sport specific are associated with high performance in a sport. Aim: The aim of this study was to examine the correlation between three parameters (power, velocity and force) measured in a golf specific rotational test and clubhead speed (CHS), ball speed (BS) and carry distance (CD) on young elite golfers. Methods: Twenty-six golfers, 16 men and 10 women, completed the study. Two tests were performed on separate occasion; one golf performance test using Trackman launch monitor and one golf specific rotational test in 1080 Quantum. CHS, BS and CD were collected in the golf performance test and max power, force and velocity in the golf specific rotational test. To study the relationship between the selected variables, spearman’s correlations coefficient (rs) was used and analyzed the total group, and in men and women separately. Results: Excellent correlation was found between max power and CHS (r=0.9, p<0.00). Good correlation was found between force and CHS (r=0.8, p<0.00). Moderate correlations were found between force and BS and force and CD and max power and CD (r=0.7, p<0.00). Poor correlations were found between velocity and CHS, BS and CD (r=0.3, p>0.50). Conclusion: This study showed that strong correlations seem to exist between power production in a golf specific rotation test and Golf performance in young elite golfers. Even force also seems to have an impact on golf performance while velocity showed little to no correlation to golf performance.

    Abstract [en]

    Background: Postpartum depression affects 13-19% of new mothers, and the consequences when treatment is not introduced in time can have permanent effects for both the mother and the child. Purpose: To describe factors that affect help seeking behaviors in new mothers with symptoms of postpartum depression. Method: A general literature study was conducted where ten qualitative articles were examined, analyzed and compiled. Results: The results of the literature study featured the themes: the impact of healthcare, the meaning of family and close relatives, the level of knowledge regarding postpartum depression, the significance of culture and expectations about maternity. Healthcare and family were described as promotive factors in women's help seeking behavior while culture, ignorance, lack of information and expectations about motherhood were prominent barriers in help seeking. Conclusion: The study showed several factors that influenced the help seeking behavior. In order to reduce stigmatization around the diagnosis and instead increase knowledge, more comprehensive information measures are needed. Nurses need to have knowledge of postpartum depression in order to understand women's help seeking behaviors and further research on the health care experience as a barrier is desirable for this only be a promotive factor in the future.

    Vigilance is a term in nursing science that plays a central part in nursing profession, but is equally as central for the affected family. Vigilance needs to be made visible as there occurs vagueness about the deep meaning of the term. The aim of this literature study was to highlight what family vigilance means in nursing tradition. The study was designed as a general literature study, where nine scientific articles with qualitative assumptions form the basis for the outcome. Hermeneutically inspired questions have been used, to generate a deep understanding about human actions. The questions contributed to a categorization of the results from the included articles, and emerged as: Feelings, which formed a main category that further formed three subcategories. The subcategories were entitled as: Protection, Commitment and Lifestyle adjustments. Protection and Commitment emerged as a way for the family to maintain their vigilance. Lifestyle adjustments arose as an inevitable result of the eternal vigilance. A further need regarding complementary research exist, concerning the deep and emotional meaning of family vigilance. By highlighting vigilance and seek understanding about the meaning of the phenomenon for the family, nursing diagnoses can be created.

    It is the nurse’s responsibility to observe, prevent and treat malnutrition. The reform of geriatric care, where the responsibility for the elderly residents has been moved from county council into municipality, has implied that access to nurses in Home Care Service is deficient in proportion to the amount of residents. This matter together with poor knowledge in nutrition among the nursing care staff, accompanied by high burden of care, have led to deficiencies in nutritional care, which can remain for a long time before revealed. The aim of the literature study was to examine the nurse’s ability to discover and treat malnutrition in elderly Home Care residents. A literature study based on 20 scientific articles was made. The nutritional knowledge among nurses and members of nursing care staff is generally low, only a small amount of the nursing care staff had knowledge in nutritional screening methods and even fewer used screening methods in practice. The individual requirement of energy for the elderly in the studies was in most of the cases not met, but increased energy intake through energy enriched food or supplement drinks, had a positive effect to counteract malnutrition. Further research in Swedish conditions, where the knowledge among the nursing care staff ought to be inventoried, is highly relevant for further development of the Home Care Services.

    Syftet med föreliggande studie var att undersöka om socialt stöd fungerar som en moderator i relation mellan stressfyllda händelser och mental ohälsa hos elitidrottare. Genom ett strategiskt bekvämlighetsurval deltog 88 elitidrottare (ålder M=17.3, SD=2.4) från södra Sverige, som representerade idrotterna fotboll (N=71) och volleyboll (N=17). Studien baserades på ett frågeformulär med följande instrument: Life Events Survey for Collegiate  Athletes (LESCA), The Athlete Received Support Questionnaire (ARSQ) och General Health Questionnaire (GHQ 12). Resultatet visade stöd för hypotesen då socialt stöd delvis modererade sambandet mellan stressfyllda händelser och symptom av mental ohälsa. Resultatet diskuterades utifrån studiens referensramar.

    Syftet (även kallat huvudtema i kategoriprofilen) med studien var primärt att (1) undersöka vilkeninställning idrottande ungdomar har till begreppet mental styrka. Sekundärt syfte som förs upp tilldiskussion var att (2) skapa en förståelse om hur de tränar och upprätthåller sin mentala styrka samt(3) vilka faktorer under träning och tävling som deltagarna anser är viktiga för att utveckla ochupprätthålla mental styrka. I studien deltog tio idrottande ungdomar (m= 16,24), 6 män och 4kvinnor inom både lag och individuella idrotter. En egen utformad semi- strukturerad intervjuguideanvändes, utvecklad med teoretiskt stöd från Jones et al., (2007) och Gucciardi et el., (2009).Resultaten i studien visade att deltagarnas inställning och kunskap om begreppet mental styrkavarierade. De flesta deltagarna hade en positiv inställning och ansåg att det var en viktig faktor i deflesta situationer inom deras idrott. Vidare visade resultaten att ungdomarna fick för liteundervisning och fokus på mentala faktorer från både skola och tränare. Såväl praktiskaimplikationer av resultat som förslag på vidare forskning inom området ges.

    Obesity is currently a major problem in children and adolescents, especially because of physical inactivity and poorer eating habits. The school is an important element in the promotion of child and adolescent habits of physical activity. School sports are also important for children and adolescent´s habits of physical activity in a lifelong perspective. The aim of the study was to highlight adolescent´s experience of school sports as motivation for physical activity. Qualitative interviews were used as data collection methods, followed by content analysis method of analysis. The sample in the study was a strategic choice, a total of nine students from grades three to two high schools in southwestern Sweden participated. The analysis of the data collection resulted in three main categories with four subcategories. The first main category was Knowledge as motivation, with subcategories Knowledge of Physical Activity and Knowledge of the health conept. The second main category was Participation and acknowledgment, with the subcategories To get positive feedback/respond and To be involved in decison-making. The last main category was Lack of personal interest. Overall theme of the results was Teachers create opportunities for physical activity. This is because the overall result of the study was that the teachers had a crucial role in the informants' experiences of physical education as a motivational and inspirational to physical activity. Continued research in this area may advantageously be to also find out the PE teachers' attitudes towards creating conditions for motivation to physical activity among students who are not interested in the topic in their teaching.

    BACKGROUND: Mental illness among young woman is a public health problem. Anorexia nervosa is categorized as a mental illness. Young woman affected by Anorexia nervosa, wich for example may be due to earlier life events that gave rise to mental imbalance in their everyday lives. OBJECTIVE: The aim of this study was to describe the socio-cultural reasons that contribute to the development of Anorexia nervosa among girls and young women 12-25 years. METHOD: The study is a literature review, based on 21 differens aricles. The method has been to look up earlier research in the area through various databases on the internet. Databases that have been used is pubmed and psycinfo. The articles were divided into different themes, with main category and subcategory. RESULTS: The results of the study was divided into two main categories each with two subcategories. Social causes for the occurrence of Anorexia nervosa: School and youth culture and the second was lack of family relationship. Media reasons in the emergence of Anorexia nervosa: Media image of the feminine ideal and negative inspiration on the internet. Socio-cultural reasons, medial reasons, family reasons, and cultural reasons. IMPLICATION: For example, one can study the various reason in a more specific level, and studying each factors separately. The study can also be used as a form of inspiration that reinforces the importance of health promotion activities for mental illness is the focus.

    Sedan 1960 talet har screening för och vaccinering mot cervixcancer blivit allt mer utbrett, trots detta drabbas fortfarande 450 kvinnor varje år av cervixcancer i Sverige. Då incidensen minskar finns risken för att kunskapen om sjukdomen minskar. Syftet var därför att belysa kvinnors upplevelser av att behandlats för cervixcancer för att öka kunskap och förståelse i mötet med kvinnorna. Studien utfördes som en litteraturstudie där 20 artiklar låg till grund för resultatet. Resultatet belyser kvinnors upplevelser av att behandlats för cervixcancer och hur det påverkade vardagen, reproduktiv och sexuell hälsa, deras syn på framtiden samt hur de upplevde sjukvården. Att ha behandlats för cervixcancer var något som drabbade hela människan, såväl fysiskt, psykiskt som sexuellt och inte bara genitalierna där sjukdomen hade sitt ursprung. Det framkom att kvinnorna ville samtala om tabubelagda ämnena men inte alltid vågade ta upp dem själva. Därför är det betydelsefullt att sjuksköterskorna ställer frågor angående de här ämnena. Omvårdnad bör grundas på evidens och därför är det viktigt att forskningen fortskrider för att optimera vården för kvinnor med cervixcancer.

    Abstract [en]

    Patients who deviate from the norms of society will be affected by nurses' negative attitudes as a result of lack of knowledge and prejudice from society. The purpose of this study was to elucidate the factors that influence attitudes nurses have in meeting with patients from the norm deviant groups. This literature studies results reflect the overall importance of knowledge and the importance of education, work environment and gender of nurses and is based on 15 scientific articles conducted in different countries. Articles consisting of 12 from quantitative approach, two from qualitative approach and one from mixed method. The results are analyzed in different stages where themes were created and a deeper understanding reached by summarizing and compiling results in article overviews. Nurses’ negative attitudes can result in poorer care and a bad attitude, which can be reduced through more education and to find out the contributing factors for nurses' attitudes may create a greater understanding and acceptance. Further research is needed to influence the factors that lead to negative attitudes.

    Abstract [en]

    The aim of this study was to examine how enterprises in Halmstad municipality, Sweden, worked with health promotion. A survey, in the form of a questionnaire was performed on large- and medium-sized enterprises in Halmstad. The respondents can be described as staff responsible for health promotion and work environmental issues. The results show that of 34 companies, 31 offer health promotion benefits, 23 actively work on improving the work environment by introducing ergonomic measures and 21 works with health services. The analysis indicates that by having these benefits and services in place, the absence due to sickness decreases while the employee’s health status increases. However, the study also found that there were difficulties in promoting health in these enterprises; mainly lack of time, economy and lack of motivation among employees. To further improve health promotion in work places we recommend educating responsible staff in particular and employees in general.

    Stressrelaterade sjukdomar är ett problem som ökar i samhället. Universitetslärare upplever ofta känslan av att inte räcka till. Syftet med studien var att se hur universitetslärare lär sig att hantera stressorer på arbetsplatsen. Vi utgick ifrån teorierna erfarenhetsbaserat lärande och KASAM (känsla av sammanhang), vi valde dessa två eftersom vi tycker att de kompletterar varandra. I tidigare forskning ses känslan av sammanhang och copingstrategier som hälsoresurser på arbetsplatsen. Vid problemlösning på arbetsplatsen kan individen genom att lösa problem få en ny erfarenhet, det är vad erfarenhetsbaserat lärande handlar om. I vår studie använde vi oss utav kvalitativ metod, då vi genomförde intervjuer av lärarna på ett högre lärosäte i Sverige. I vår studie deltog sex universitetslärare. I vårt resultat framkom det generella motståndsresurser (GMR) som exempel ta hand om sig själv, ha en egen copingstrategi, goda relationer med kollegor och gränssättning mot för mycket arbete. Vi ser i vår studie att individer utvecklar olika copingstrategier. Exempel på copingstrategier som resultatet i vår studie visar var att tänka positivt och bryta ner saker till mindre delar. Alla hade olika sätt att hantera stress och det handlar om vilka erfarenheter de har skapat sig längs vägen. I diskussionen väljer vi att bland annat diskutera likheter och skillnader mellan copingstrategier mellan vår studie och tidigare forskning. Vi ser även närmare på likheter och skillnader på våra teorier som vi valt för studien.

    Abstract [en]

    The purpose of this study was to examine the sources to self-efficacy among individual sportathletes based on self-efficacy theory and the high performance pyramid.  The objectives were to examine: (A) sources individual sportathletes uses in high self-efficacy (B) sources of individual athletes in low self-efficacy (C)  the relationship between self-efficacy and performance. Ten semi-structured interviews were conducted with ten individual athletes (five men and five women) in age 18 - 27. The results showed that past experience was the major source of high self-efficacy with expressions such as "trained well before" and "good preparation" used the most by the participants. "Performance anxiety" and "external pressure” were the largest source of low self-efficacy. Examining the relationship between self-efficacy and performance revealed two pattern, high self-efficacy in 90 % cases is associated with successful performance and low self-efficacy in 80% cases is associated with poor performance.

    Abstract [en]

    Migraine is a recurrent headache that occurs in periods of sudden attacks and is regarded as one of the 20 most disabling disorders in the world. It is the responsibility of the nurse to promote the health of patients with migraine by educating about migraine and by providing vital information about non-pharmacological methods that has beneficial effects on migraine. The aim of this literature study was to describe non-pharmacological methods on adult migraine patients. The result of this literature study was based on 13 scientific articles that generated three themes: acupuncture, knowledge about trigger factors and lifestyle changes. The result showed that acupuncture, knowledge about migraine triggers and lifestyle changes are favourable alternative treatments in migraine health promotion as they demonstrated significant reduction of migraine attacks. To attest its validity further research regarding the use of non-pharmacological methods on migraine is necessary. Specific guidelines regarding migraine and adequate available treatment methods in healthcare settings would be of great importance. We encourage additional education on migraine and nonpharmacological, preventive methods to be integrated in nursing schools.

    Abstract [en]

    The purpose of this study is to examine the extent to which children's physical activity during school  hours  are  met  in  relation  to  today’s  health  recommendations.  This  study  was  a quantitative  research  method  through  study  with  pedometers  and  observation.  The  main instrument was pedometers which the pupils wore during one day at school. The study group consisted of 74 students, made up of 36 boys and 38 girls from the 4th grade in four schools in  southern Sweden. The  results demonstrate  that  the boys were physical active 47-50 % of what  they should and  the girls 33-36 % of what  they should according  to recommendations. The boys  took about 3000 more  steps  than  the girls.  In our  study only one boy  reached  the recommended  amount  of  steps  per  day,  but  no  girl  did. The  conclusion  is  that  the  pupils  are  inactive  during  their  time  in  school.    There  are differences  in  boys'  and  girls'  physical  activity,  both  within  each  school  and  between  the schools. The observation showed that the girls stand still more than the boys during break and the girls are generally less active than the boys at all four schools.

    Abstract [en]

    Patient centered care is described as seeing the entire person and the person’s right to mutual respect, understanding and empowerment during care. The implementation of patient centered care is advocated to meet the expectations of high quality healthcare where patients experience safety and a trusting environment. Evaluation of patient centered care is important to demonstrate how the basic needs of trust and safety are provided for in a transfer to a different care model. The aim of this study was to describe factors that influence trust and safety when the patient is cared for in a patient centered manner. A systematic literature review was undertaken and six qualitative and four quantitative articles published between 2009-2014 were analysed based on an inductive method. Factors that influenced trust and confidence in patient centered care emerged from the results and were described in three categories: the nurse-patient relationship, sharing of information and knowledge, and organisational factors. The overall results shows that patient centered care generates trust and safety for the patient. Facilitators are a mutual, respectful nurse-patient relationship, the gaining of knowledge about the disease to be able to participate in care and decision-making. A knowledgeable, skilled nurse who contributes to continuity and accessibility generates trust and safety. Further research is needed to evaluate the effects of patient centered care in clinical practice. Gaining awareness of facilitators that create trust and confidence for the patient may contribute to quality improvements when developing patient centered care.

    Abstract [en]

    Background

    Mental illness is an increasing health problem globally. However, many individuals do not seek health care although evidence-based care is available. Research has shown that self efficacy is associated with various health outcomes and it is of importance to investigate if it also is associated mental illness. Early detection promotes recovery and decreases suicide risk.

    Aim

    The aim of this study is to investigate whether low levels of self efficacy is associated with a higher degree of mental illness and whether level of self-efficacy influence health seeking behaviour.

    Methods

    This is a cross-sectional study based on data from the Health Assets study, with data collected in 2008 in West Sweden. The study population is a randomly selected population sample of 4027 individuals, aged 18–65 years. Data collection was done by a postal questionnaire and the response rate was 50.4%. Bi-and multivariate analyses were employed to investigate associations and results were stratified on sex, age, civil status, education and social support.

    Results

    A total number of 1361 (36%) out of 3811 individuals answered ‘Yes’ on the question ‘Have you ever felt so mentally ill that you had (felt a need) to seek care’. A total of 33% of the women answering yes were found in the lowest quartile of the general self efficacy scale, 20% in the highest quartile. Corresponding figures for men were 30 and 23% respectively. The most common reason stated for not seeking health care was a belief that the mental health problem would disappear by itself. Others reasons mentioned were beliefs that health care would not help, they did not know were to go or they felt ashamed for showing others they suffered from mental illness.

    Conclusion

    Mental illness is a serious health problem and access to care needs to be improved. Health promotion should also include individual traits/characteristics such as self efficacy and health-seeking behaviour.

    Regional differences in Sweden in the prevalence of disability pension with a psychiatric diagnosis are unexplained, in spite of the significant impact on the population's health, rehabilitation systems, and the health care system. The purpose of this study was to describe the pattern of disability pensions with a psychiatric diagnosis and to analyze the impact of age and gender. We examined the incidencerates in one urban and one semi-rural region and compared these to national rates. The study sample was drawn from employed persons between 16-64 years of age who, because of their sickness insurance coverage, would be eligible to access disability pensions should it be necessary. Analysis of annual incidences and standardized morbidity ratios were made for 1980, 1985, 1990, 1995, and 1998. Data ondisability pension cases were collected from the National Social Insurance registers. In the urban region we found that the proportion of men and women clearly outnumbered the national average: approximately twice the number of persons between 16-64 years of age with apsychiatric diagnosis were receiving a disability pension. In the semi-rural region there were fewer men overall on disability pensionswith psychiatric disorders, but in 1980, 1985, and 1995 women clearly outnumbered men. Access to psychiatric care, unemployment, alcohol dependence, and previous sickness absence are suggested as possible factors that might affect the rates of disability pension in different geographical settings. © 2006 IOS Press.     There is growing concern about alcohol problems in low- and middle-income countries. More research is required, particularly among the younger generation. The aim of this study was to investigate the prevalence of alcohol disorders and associated socioeconomic characteristics among young men and women living in the Eastern Cape Province, South Africa. This was a cross-sectional population-based study of 977 participants (52% male and 48% female) aged 18–40, the majority of whom lived in low-income areas. Data collection was carried out in 2012 by trained fieldworkers. The Mini International Neuropsychiatric Interview (DSM-IV) was used to investigate the prevalence of alcohol dependence (increased tolerance to alcohol, failed attempt to cut down, risk of physical and mental effects) and alcohol abuse (harmful use, consistent intoxication, risk behaviour, physically hazardous, social problems). A high 12-month prevalence of alcohol dependence was found (26.5% in total; 39.0% among men and 19.1% among women) as well as of alcohol abuse (9% in total; 19.0% among men and 6.0% among women). Few socioeconomic differences emerged among the men, except older men (OR 1.94, CI 1.11–3.42) and those supported by social grants (OR 2.28, CI 1.06–4.93), who presented higher odd ratios for alcohol dependence than the reference groups. Among the women, more differences emerged: women who were widowed/single (OR 2.35, CI 1.20–4.62), had no education (OR 3.41, CI 1.04–11.21), had a low income (OR 3.26, CI 1.55–6.80) and had no social support from friends when ill presented higher odd ratios (OR 1.73, CI 1.07–2.80). In the adjusted model, marital status and low income remained statistically significant. With regard to alcohol abuse, fewer socioeconomic differences emerged. Interventions need to address the early onset of alcohol misuse in order to meet both current needs and long-standing mental and physical illness. © 2017 John Wiley & Sons Ltd

    Antalet elever med psykisk ohälsa har ökat. En tredjedel av skolsköterskors arbetstid ägnas åt elever med symtom på psykisk ohälsa. Syftet med studien var att belysa skolsköterskors erfarenheter i mötet med elever i årskurs fyra till nio som visar symtom på psykisk ohälsa. Metoden var en integrativ litteraturstudie för att kunna förstå och förklara ett fenomen. Skolsköterskors erfarenheter i mötet med elever var att de förvaltade ett förtroende, att inte räcka till och att kunna göra skillnad. Studien visade att skolsköterskor upplevde en rädsla för att inte räcka till för elever med psykisk ohälsa. Detta kunde bero på en bristande kunskap i att hantera dessa elever och det fanns en rädsla av att inte nå de elever som verkligen behövde hjälp. Skolsköterskor kunde känna en otillräcklighet i att hjälpa eleverna att själva förstå och hantera sin situation. Mer forskning behövs för att undersöka hur skolmiljön kan förändras för att minska risken för psykisk ohälsa. Fler kvalitativa studier behövs som belyser skolsköterskors men även elevers upplevelser av psykisk ohälsa i skolmiljön.

    To become seriously ill and cared for in an intensive care unit is for many an unpleasant experience. Monitoring and treatment of vital functions takes place around the clock in an environment that is foreign to the patient. Many patients, due to drug treatment and difficulties in communication, can suffer from memory loss and hallucinations. As an act of caring, the nurse can write a diary throughout the hospital stay. The aim of this study was to focus on the patient's experience of reading a personal diary of their time in the intensive care unit and assess how it can affect the patient's recovery. Twelve articles were analyzed and summarized in five categories: Experience of receiving and reading the diary, The importance of feeling cared for, To gain insight into their own vulnerability, To interpret memories and fill memory gaps and The diary's importance for recovery. Many patients feel unpleasant after a period of intensive care. Results have shown that the diary helped them cope with time afterwards as it filled memory gaps which supported a faster recovery. Patients felt a greater level of care and received the diary as a gift. To optimize this support for the patient, further research on what the diary should contain would be of interest.

    Kronisk obstruktiv lungsjukdom är en progressiv sjukdom som kännetecknas av kronisk luftvägsobstruktion. Den främsta orsaken till sjukdomen är rökning. Andra orsaker till sjukdomen kan vara luftföroreningar samt ärftlighet. Tidigare forskning visar att kronisk sjukdom innebär begränsningar som ger upplevelsen av förlust, vilket ger upphov till lidande. KOL leder till andnöd vilket ger begränsningar i livet. Andning är förknippat med liv och vid andnöd kan livet upplevas hotat. Personer som drabbas blir beroende av att ha kontakt med hälso- och sjukvården och i de senare stadierna av sjukdomen är även behovet av omvårdnad stort.  Syftet med studien var att beskriva upplevelsen av att leva med KOL. Litteraturstudien baserades på 15 vetenskapliga artiklar. Resultatet innefattade fyra teman: att uppleva andnöd; att uppleva sociala och personliga förändringar; att hantera livet med KOL; att leva med en självförvållad sjukdom. Livet med KOL präglades av känslor så som oro, ångest, förlust, begränsningar, skam och skuld. Livet för personer med KOL kan upplevas lättare att hantera om en ny mening med livet hittas. Bemötandet från hälso- och sjukvården spelar in på hur personer med KOL upplever sin sjukdom och genom förståelse för hur personer med KOL upplever sjukdomen kan ett gott bemötande från sjuksköterskan samt personcentrerad omvårdnad ges. Mer forskning behövs så att sjuksköterskan får ökad förståelse för hur det är att leva med KOL och därigenom ge bästa möjliga omvårdnad.

    Tidigare forskning visar att smittskyddsisolering kan ha negativa effekter på patienters välmående. Syftet med denna studie var att belysa patienters erfarenheter av att vara smittskyddisolerade på ett sjukhus. För att få en uppfattning om det vetenskapliga läget genomfördes en litteraturstudie med induktiv ansats där nio artiklar granskades och analyserades. Genom färgkodning framkom olika teman och dessa bildade sedan tre olika rubriker. Dessa rubriker var: Att vara ensam, Behovet av information och Begränsningar i vårdmiljön. Resultaten visade att smittskyddsisolerade patienter blir ensamma. Ensamheten berodde främst på brist på besök från närstående och vårdpersonal. Många saknade också möjligheten att umgås och prata med andra patienter. Patienter hade otillräcklig kunskap om sin situation eftersom informationen från vårdpersonalen varit knapphändig. Bristen på kunskap orsakade förvirring hos patienter, dessutom uppstod ilska gentemot vårdpersonalen. Vårdmiljön ledde till begränsningar både fysiskt och psykiskt. Det ledde också till minskad grad av självbestämmande. Vid smittskyddsisolering behöver patienter få mer social stimulans. Vårdpersonal bör vara mer närvarande och bli bättre på att informera patienter.

    Kommunikation är en central del i distriktssköterskans arbete och forskning visar att

    bristande kommunikation och informationsöverföring mellan distriktssköterska

    och omvårdnadsteamet är riskområden för patientsäkerhet. Syftet med studien var att

    beskriva distriktssjuksköterskans erfarenheter av kommunikation och informationsöverföring

    inom omvårdnadsteamet. Fyra intervjuer genomfördes och datan analyserades med kvalitativ innehållsanalys. Tre kategorier framkom: tillämpa kommunikationsstrategier, verka för samarbete och arbeta med organisationsansvar. Distriktssköterskan använde olika strategier i kommunikationen med omvårdnadsteamet för att skapa goda relationer. Hög kontinuitet mellan distriktssköterskan och omvårdnadsteamet ger förutsättningar för ett effektivt samarbete vilket ökar patientsäkerheten. Organisationen var en betydande påverkansfaktor. Förbättringsarbete inom klinisk verksamhet kan uppnås genom att uppmärksamma kommunikation som ett komplext område. Vidare forskning krävs om kommunikation mellan distriktssköterskan och omvårdnadsteamet.

    Genom hela livet har nutrition en stor betydelse för människan. I palliativ vård prioriteras inte alltid nutrition trots att det är viktigt för patienten. Sjuksköterskans roll är att stötta patienten och gynna nutritionen för att på så sätt främja välbefinnande och lindra lidande. Syftet med studien var att belysa nutritionen i ett palliativt skede. Studien är en litteraturöversikt där 13 artiklar användes, artiklarna söktes i databaserna PubMed och Cinahl. Resultatet delades in i tre teman: en symbol för livet, ett etiskt dilemma. Nutritionen hos de palliativa patienterna har flera fördelar, den kan förlänga livslängden, förbygga undernäring, ge patienten komfort och lugna oroliga närstående. Nutritionsfrågan kan lätt bli ett etiskt dilemma då patienten, närstående och vårdpersonal har olika syn på hur behandling bör ges, olika kunskapsgrad ligger ofta till grund för oenigheten. Kommunikationen är en viktig faktor för att alla berörda ska känna sig delaktiga. Trots att nutritionsbehandlingen har få medicinska effekter är det en viktig del för patienten och närstående, nutritionen kan förebygga känslor av övergivenhet, hopplöshet och bristande förtroende hos både patient och närstående. Sjuksköterskan behöver god kunskap och hög kompetens för att kunna vara mer delaktig i beslut som tas om patientens nutrition. Det krävs mer forskning inom området palliativ vård som inriktas mot nutrition för att kunna tillgodose krav från patienter och närstående.

    Nurses are experiencing huge demands and big loads in their daily work, both from patients and from colleagues and from themselves so that they often feel inadequate. This great physical and psychological workload means that nurses are concerned about the quality of care which gives rise to the moral distress. Nurses are experiencing this moral distress at greater extent than other professions and is a major reason why nurses quit their jobs. In previous research, few studies have moral distress as their main purpose. Therefore this general literature review intends to look more closely at what the nurse is experiencing in his or her moral distress. The purpose was to describe the nurse's experiences of moral distress. For this purpose, 13 qualitative research articles were selected and the results revealed six categories: Powerlessness, Frustration, Agony, Anger, Self-doubt and Guilt. Powerlessness has a significant role and contributed to many of the other emotions listed. Powerlessness is mainly due to the nurse not feeling involved in decision-making regarding the patient. To reduce the nurse's moral distress collaboration between professions could be encouraged. Further Swedish research could be needed to identify whether moral distress is a contributing factor to the current nursing shortage.

    BACKGROUND: It is not known whether snuff (moist smokeless tobacco) affects disease activity in rheumatoid arthritis (RA).

    OBJECTIVE: This study aims to study the effect of snuff on disease activity and function in Swedish patients with early RA.

    METHODS: Between 1992 and 2005, 2800 adult patients were included in the Better Anti-Rheumatic FarmacOTherapy (BARFOT) early RA study in Sweden. Disease Activity Score 28 joints (DAS28), Health Assessment Questionnaire, visual analog scale for general health, and drug treatment were registered at inclusion and at follow-up after 1, 2, and 5 years. European League Against Rheumatism response and remission criteria were applied at 1 year. In 2010, a self-completed postal questionnaire was sent to 2102 patients in the BARFOT study enquiring about lifestyle factors such as smoking and use of snuff. Three controls for each patient using snuff were identified.

    RESULTS: Fifty-one patients who used snuff were identified, together with 145 controls. When we adjusted for socioeconomic class, disease duration, and previous antirheumatic medication, the snuff users had lower DAS28 values at up to 6 months of follow-up than patients who had never smoked, and they had lower DAS28 values than previous smokers at up to 2 years of follow-up. No effect of snuff use on European League Against Rheumatism response was seen at up to 1 year.

    BACKGROUND: There have been no reports on the effect of immigrant status and socioeconomic status on outcome in rheumatoid arthritis (RA) in Sweden.

    METHODS: Between 1992 and 2006, 2,800 patients were included in the BARFOT study on early RA in Sweden. Disease Activity Score 28 joints (DAS28), Health Assessment Questionnaire (HAQ), treatment and European League Against Rheumatism (EULAR) response criteria were registered. In 2010, 1,430 patients completed a questionnaire enquiring about demographics and lifestyle factors.

    RESULTS: One hundred and thirty-nine of the 1,430 patients (9.7%) were immigrants. At baseline immigrants had higher mean HAQ (1.2 vs 0.97 for non-immigrants, p=0.001), DAS28 (5.6 vs 5.2, p=0.000), visual analog scale (VAS) pain (56 mm vs 45 mm, p=0.000), VAS global health (53 mm vs 44 mm, p=0.000) and tender joint count (TJC) (10 vs 8, p=0.000). These differences persisted for up to 2 years of follow-up (for HAQ, for up to 8 years of follow-up). Immigrant status did not have any effect on swollen joint count (SJC), ESR, CRP or EULAR response. Socioeconomic class did not have any effect on treatment or outcome.

    CONCLUSIONS: Immigrants scored worse in pain, function and TJC for up to 2 years of follow-up, but they did not differ from non-immigrants in objective measures of inflammation or EULAR outcome. This could be due to different perceptions of health and pain and/or the stress of immigration. Socioeconomic class had no effect on treatment or outcome, and this could be due to the relatively egalitarian society in Sweden.

    OBJECTIVE: To study the prevalence of chronic widespread pain (ChWP), chronic regional pain (ChRP), and fibromyalgia in patients with early rheumatoid arthritis (RA) followed for 5 years after inclusion, and to study the effect of pain on measures of disease activity and function.

    METHODS: A questionnaire was sent to 1910 patients participating in the Better Anti-Rheumatic Pharmacotherapy study. The responders (73%) were divided into 3 groups according to the reported pain duration and distribution - patients having no chronic pain (NChP), ChWP, and ChRP. Outcome measures were the 28-joint Disease Activity Score (DAS28), the Health Assessment Questionnaire (HAQ), and C-reactive protein (CRP).

    RESULTS: Thirty-four percent of respondents reported ChWP, 46% ChRP, and 20% NChP. Patients reporting ChWP were more often women and had more pain and tender joints at inclusion. From 6 months to 5 years of followup, mean DAS28, visual analog scale (VAS) pain, VAS global health, and HAQ were significantly higher in the ChWP group than in the other groups. However, all groups showed a similar pattern in swollen joint count, erythrocyte sedimentation rate (ESR), and CRP. From 12 months the ChWP group was treated with prednisolone to a greater extent than the ChRP group, and it had a rate of treatment with disease-modifying antirheumatic drugs similar to that of the ChRP group.

    CONCLUSION: ChWP is a common feature in RA, more associated with high values for variables related to pain such as the DAS28 and HAQ than to indicators of ongoing inflammation such as swollen joint count, ESR, and CRP. Patients with ChWP should be identified so that adequate treatment also of the noninflammatory pain may be instituted.

    Background: Studies of alcohol use in patients with rheumatoid arthritis are sparse and studies of why patients choose to stop drinking alcohol in particular.

    Objectives: The aim of the current study was twofold: first to identify patients with RA who stopped drinking alcohol and compare those to patients drinking alcohol, and second, to explore reasons to stop drinking alcohol.

    Methods: In 2010 a self-completion postal questionnaire was sent to all 2,102 prevalent patients in the Better anti-rheumatic farmacotherapy (BARFOT) study enquiring about disease severity, physical function (HAQ) and health related quality of life (EQ5D), pain, fatigue, patient global assessment (PatGA) and lifestyle factors e.g. alcohol. The questions assessing alcohol included the question “Have you stopped drinking alcohol?” and an open question “Why have you stopped drinking alcohol?” A mixed method design was used and 1512 patients had answered the alcohol questions and was included in the study of those 86 had stopped drinking alcohol. Seventy-one patients answered the open question and their answers were analyzed with qualitative content analysis (1).

    Results: Comparing patient with RA using alcohol or not, the patients who stopped drinking alcohol was older median age (min-max) 69 (36–90) vs. 66 (23–95), p=0.011, more men 42% vs. 29%, p=0.015, had worse physical function, median HAQ (min-max) 0.50 (0–3.00) vs. 1.00 (0–2.75), p<0.001, worse health related quality of life, median EQ5D (min-max), 0.69 (-0.59–1.00) vs. 0.76 (-0.02–1.00), p<0.001, worse self-perceived health, median PatGA (min-max) 5 (0–10) vs. 3 (0–10), <0.001, more pain, median (min-max) 5 (0–10) vs. 3 (0–10), p<0.001, and more fatigue median (min-max) 6 (0–10) vs 4 (0–10), p<0.001. There were no differences between the groups regarding disease duration, swollen and tender joints. The qualitative content analysis resulted in five categories describing the reasons for patient with RA to stop drinking alcohol: disease and treatment, health and wellbeing, work and family, faith and belief and dependences and abuse.

    Conclusions: Patients with RA who stopped drinking alcohol have a lower physical function, health related quality of life, self-perceived health and more pain and fatigue comparing to patients with RA drinking alcohol. The reasons to stop drinking alcohol were of different nature such as medical, physical, mental, social and spiritual

    Relatives to patients receiving palliative care are subjected to all kinds of different pressures. The duty of the nurse is to assess and form a complete picture of the support the relatives need, and what kind of support that she can provide for them.

    The purpose of the study was to illustrate the different kind of support the relatives to patients in palliative care have, and what the nurse can do for them.

    The method used was a literature study based on 12 research articles, both qualitative and quantitative.

    The need for support varied over time and depended on how the relative’s relationship had been to the patient prior to illness. Many relatives found it very helpful to be able to talk to others with similar experiences. Professional help worked as a complement. One important task for the nurse was to teach the relatives about the specific care problems that could arise, including how to operate certain medical equipment. The nurse’s aim was to be able to give individual support that was specifically adapted to each relative’s need. In the result four categories emerged; the need of emotional support, the need of confirmatory support, the need of informative support and the need of instrumental support.

    Future research may focus on if the need of support to relatives differs depending on if the patient is treated in non-institutional care or in hospital care, if the patient becomes ill rapidly or slowly, and how the health care system can integrate the relatives in the care in a better way.

    Sexuality is a fundamental part of being human and sexual health is an important part of holistic care. More women than men suffer from sexual dysfunction. Nevertheless, nurses rarely address women's sexual health. The purpose of the study was to highlight women's experiences of sexual dysfunction. In response to the purpose, a general literature study was conducted. A systematic literature search resulted in 14 results critically reviewed. Data processing revealed three categories: Inability to live up to social expectations, The impact of dysfunction on health and The need for support. The result showed that women with sexual dysfunction experienced social expectations that with sexual dysfunction became impossible to live up to. The women expressed that dysfunction affected health and there was also a need for support from both the partner and the healthcare sector. The healthcare confirmation of dysfunction was described to reduce the perceived expectations. Redefining sexuality could be supportive in finding new ways of sexual health. Healthcare is considered to be the most reliable source of information and women experience a high level of confidence in the nursing profession. Based on the results of this study, nurses are expected to have great opportunities to support women for sexual health. Nurses need to routinely raise the issue of sexuality. Sexual health also needs to be an important part of nursing education so that sexuality becomes as obvious as addressing, for example, tobacco habits and sleep.
