Abstract [sv]

Bakgrund: Smärta är ett av de mest förekommande symtomen hos patienter som söker vård på en akutmottagning. Smärta är en subjektiv upplevelse, därför är smärtbedömningen en komplex omvårdnadshandling som ställer stora krav på sjuksköterskors kompetens. Syfte: Var att belysa sjuksköterskors erfarenheter av att bedöma smärta på en akutmottagning. Metod: En allmän litteraturstudie genomfördes med resultat från nio vetenskapliga artiklar som bearbetades med stöd av innehållsanalys. Resultat: Innehållsanalysen resulterade i tre huvudkategorier: Dissonans mellan objektiva fynd och subjektiva tecken, erfarenheter av smärtskattningsverktyg vid smärtbedömning samt strukturell och organisatorisk påverkan vid smärtbedömning med respektive subkategorier. Sjuksköterskor beskrev att smärtbedömningen var en komplex omvårdnadshandling som påverkades av såväl strukturella som organisatoriska faktorer. Vidare påverkades smärtbedömningen av såväl sjuksköterskors som patienters förmåga att tolka smärtskattningsinstrument. Konklusion: Studiens resultat visade att faktorer som påverkade sjuksköterskors smärtbedömning kunde kopplas till kunskap, bristande tillgång till riktlinjer, organisatoriska och strukturella faktorer samt komplexiteten av att bedöma smärta som inte alltid kan tolkas av mätbara tecken.

Abstract [en]

Background: Pain is one of the most common symptoms in patients seeking care in an emergency department. Pain is a subjective experience and therefore the pain assessment is a complex part of nursing which places great demands on the competence of the nurse. Aim: was to elucidate nurses’ experiences of pain assessment in an emergency department. Method: A general literature study was conducted. The results from nine scientific articles were processed with the support of content analysis. Result: The content analysis presented three themes: Dissonance between objective findings and subjective signs, experiences of pain assessment tools in pain assessment, and structural and organizational impact of pain assessment with each subcategories. Nurses described that the pain assessment was a complex part of nursing which was affected by both structural and organizational factors. Furthermore, the pain assessment was affected by both the nurses and patients' ability to interpret the pain assessment tools. Conclusion: The results of the study showed that factors which influenced the nurses pain assessment could be linked to knowledge, lack of access to guidelines, organizational and structural factors, and the complexity of assessing pain that cannot always be interpreted by measurable signs.
