Abstract [sv]

En väl fungerande kommunikation är en grundförutsättning för god palliativ omvårdnad. Det kan därför vara av intresse att beskriva sjuksköterskors egna erfarenheter av att kommunicera. Syftet var attgenom en allmän litteraturstudie beskriva sjuksköterskors erfarenheter av kommunikation med patienter och närstående inom palliativ omvårdnad.Tolv resultatartiklar framkom. Engagemangidentifierades som ett övergripande tema med tre kategorier: Omfattande ansvar, Känslomässig utmaningoch Personlig utveckling. I resultatet sågs att kommunikationen med patient och närstående inom palliativ omvårdnad innebar ett stort engagemang för sjuksköterskorna. Sjuksköterskorna ville ta ansvar för kommunikationen då den låg till grund för den personcentrerade omvårdnaden. Sjuksköterskorna strävade efter ärlighet i kommunikationen, vilket även innebar känslomässig utmaning. I relationerna utmanades sjuksköterskorna att balansera personlig närhet med professionell distans. Kommunikationen kunde vara emotionellt påfrestande, men reflektion gav sjuksköterskorna möjlighet att bearbeta upplevelserna. Reflektionen utvecklade även sjuksköterskornas erfarenheter till djupare kunskap. Kommunikationen främjade sjuksköterskornas personliga utveckling och de kunde därför också betrakta sitt arbete som ett privilegium. Sjuksköterskor inom palliativ omvårdnad känner ett behov av ökad kunskap och klinisk verksamhet. De behöver tid och stöd från organisationen för bearbetning av upplevelser. Ytterligare forskning behövs kring hur sjuksköterskors säkerhet i kommunikationen med patient och närstående kan stärkas.

Abstract [en]

A well functioning communication is a prerequisite for a good palliative care. Consequently it can be of interest to describe nurses´ own experiences of communicating. The aim of this literature review was to describe nurses´ experiences of communicating with patients and families in palliative care. From the twelve analysed articles the overall theme Commitmentemerged and also three categories: Comprehensive responsibility, Emotional challengeand Personal development. The nurses´ communication with patients and families in palliative care implied an extensive commitment. The nurses wanted to take responsibility for the communication as it formed the basis for the person centered care. The nurses had an ambition to communicate honestly, an ambition which turned into an emotional challenge. In the relationships the nurses were challenged to balance personal closeness and professional distance. The communication was emotionally demanding, but reflection gave the nurses a possibility to process their experiences. Reflection also consolidated their experiences to knowledge. The communication promoted the nurses´ personal progress and also made them regard their work as a privilege. Nurses in palliative care need more knowledge and clinical practice. They also need more time and support from the organisation in order to process their experiences. Further research is needed regarding the strengthening of nurses´ security in communicating with patients and families.
