Cardiac rehabilitation care efforts for patients with ischaemic heart disease: a 5-year comparative study at 18 hospitals in Sweden

Cardiac rehabilitation is described as a life-long process, starting at the onset of the disease and continuing throughout life. The majority of cardiac rehabilitation programmes are multiphase and interdisciplinary, including physical exercise, education and counselling. The aim of the study was to compare different cardiac rehabilitation approaches regarding patients with ischaemic heart disease and their next-of-kin during a 5-year period in five counties in Western Sweden. The method of data collection was questionnaries, which were answered on two occasions, in 1993 and 1998, by a person appointed by the respective department of medicine. The questions dealt with the cardiac rehabilitation organisation, which target groups were invited to participate, what rehabilitation efforts were offered, and which healthcare professionals were involved in the cardiac rehabilitation, In all, 18 hospitals (2 university, 6 county and 10 district county hospitals) took part in the study. The results show that patients with myocardial infarction and their next-of-kin were offered a well-functioning cardiac rehabilitation care, both in 1993 and in 1998, at the university, county and district county hospitals. For patients, who had undergone a bypassoperation, and their next of kin, cardiac rehabilitation was only available to a lesser extent, while patients with angina pectoris and their next-of-kin had the least possibilities of receiving cardiac rehabilitation during the 5-year period. No cardiac rehabilitation programmes were specially designed for women and an age limit existed in some cases. The cardiac rehabilitation care effort, which had been developed the most during th period of measurement, provided education about stress management. Dieticians, cardiologists, physiotherapists and nurses were the healtcare professionals most frequently included in the cardiac rehabilitation teams at the university and county hospitals, both in 1993 and in 1998. The corresponding healthcare professionals at the county hospitals were cardiologists, social workers, clergymen, physiotherapists and nurses. Suggestions for further research are to develop individual cardiac rehabilitation programmes and to carry out a randomised study of patients with angina pectoris and their next-of-kin as well as of female patients with ischaemic heart disease.
